Church Health Assessment Tool

The CHAT Survey

Having your finger on the pulse of church health is an important discipline for every church leader. But it’s difficult to gather much more than anecdotal feedback from those closest to you. Now you can get widespread anonymous feedback from your entire church using an innovative, web-driven church survey from Leadership Transformations. CHAT is a convenient, affordable, user-friendly online church survey that allows you to get an accurate baseline data pool using 10 proven church health categories (view our church health model) based on the Baker best-seller, Becoming a Healthy Church. In as little as 30 days, you can collect and study crucial feedback from your congregation and leadership team.

Never before has conducting a church survey been so easy. CHAT does all the work for you but keeps you in control of the process and sends the end report right to your email inbox. The report is designed to help you frame your strategic decision making for increased ministry effectiveness. It’s principle-based not program-based, so it won’t interfere with your current ministry programming. CHAT does all the work of data collection and tabulation but you are in the driver’s seat! You can even track your progress from start to finish via our Pastor’s Console. Best of all, you have the benefit of outside expertise without the expense of a consultant. If you’d like one of our trained Church Health Coaches to come along side you, we’re happy to provide one. The point is, you determine what you’d like assistance with and what you and your team can handle on your own.

CHAT is a 72-question assessment covering cross-denominationally applicable church health principles. You can also add custom questions of your own. Since the assessment is web-based, participants can take the survey from the convenience of home or office (paper option also available) in a 20-25 minute timeframe. Responses of the congregation and leadership team are tracked separately for direct comparison in the report. At the end of the survey dates that you choose, the CHAT system begins crunching your data overnight and your comprehensive but easy-to-read CHAT report will be delivered right to your email inbox the following morning. Identities of the respondents are kept strictly anonymous, though you will be provided with demographics of the participants such as age, gender, marital status, ethnicity, education, and years attending your church. If you’d like additional assistance at the end of the process, you have the option of working with one of our qualified Church Health Coaches via a convenient and affordable telecom package. We keep the costs down by providing coaching by phone rather than putting coaches on airplanes.

We periodically hold online trainings via Zoom to certify and empower individuals who are passionate about church health to be equipped to utilize CHAT in their own settings.

We are team players for the health of the local church! We offer a no-cost CHAT Partnership agreement which allows denominations and church networks to add CHAT to their own ministry toolbox and offer CHAT to their member churches at a discounted price.

“Leadership Transformations is to be commended for their commitment to a solid biblically-based analysis of the local church. The church is often easily distracted by culture, politics, etc. I highly recommend CHAT to all churches as a strategy to stay focused on God’s mission. It is not a pass or fail assessment. It is a tool to help churches and pastors thrive in difficult times. Don’t miss out on this easy to use tool to help your church become a place of transformation and ministry.”

Mitzi Mak

Selah-West Faculty & Emmaus Faculty

Mitzi started her professional life as a high school social studies teacher. She and her husband Jerry then served cross-culturally for ten+ years, living abroad first in India and then Kurdistan, N. Iraq. In addition to being a Spiritual Director, she now serves as a Formation and Care pastor in her local church in Houston, TX. She has graduated from LTI’s Selah Spiritual Direction training as well as LTI’s Emmaus Formational Leadership Program.

Mitzi enjoys engaging conversation, reading fiction, doing jigsaw/crossword puzzles, ocean gazing and exploring the world with Jerry through food and travel.

God has two main callings in Mitzi’s life: to care for those who care for others and to be a guide in helping others have a healthy relationship with the Trinity – recognizing God’s loving presence and activity in their lives and how to faithfully respond.

Selah was a transformative experience for me – allowing the contemplative within to emerge and to beautifully co-exist with my extraverted personality.
